New Patient Registration
The practice has an open list and is registering new patients whether new to the area or not.
You can register online or by asking for a registration form at reception.
Trinity Medical Centre welcomes new patients from within our catchment area (opposite). When you register at the surgery we will need information that will help us trace your medical records from your previous doctor. We will also need your address, post code and telephone number.
Your medical records may take a while to arrive from your previous doctor therefore we will ask you to complete a 'New Patient Questionnaire' which will provide us with some basic information about your health. You will also need to have a new patient check with one of our nrses so that we are aware immediately of your medical history and current medication. The receptionist will arrange this for you when you register.

Change of Personal Details
If you change your name, address or telephone number please give full details of the change at reception as soon as possible. Please be aware we can only make a change of either of the above if you provide us with proof of address/ or name change. If you move away from the practice area you will be asked to find a doctor in your new area.
