Meir Primary Care Network – Social Prescribers

Social Prescribing is a way for GPs and other health professionals to refer people to local, non-clinical services that support health and wellbeing.

Our Social Prescribing Link Worker, Julie, works with anyone over the age of 18 in the local community. She is available:

  • Mondays – at the practice
  • Tuesdays to Thursdays – by telephone appointment
  • Wednesdays - 9.30-12 at Wellbeing Wednesday at Rowan Village, George Street, Meir. Peer Support Group (no appointment needed)
  • Thursdays - 9.30-12 at Jolly Potters Over 50's young at heart group - Forrester Bros Community Room Longton. Peer Support Group (no appointment needed)

Other Social Prescribers are available:

  • Mondays & Fridays, 10am–12pm – at The Community Lounge (Meir) for drop-in support (no appointment needed)

Peer Support and Self-help Groups

Our Social Prescribers offer many support Groups and activities throughout the local community.  All of our groups are FREE to attend and you don't need to book.
