Our Care Home Service provides high-quality, personalised care for residents in local care homes. We work closely with care home staff, families, and healthcare professionals to ensure residents receive the support they need.

Weekly Virtual Ward Rounds
Our clinical team holds a virtual ward round every week to review residents’ health, manage medications, and respond to any concerns.
Monthly Face-to-Face Ward Rounds
Once a month, we visit each care home in person to carry out a full review and provide hands-on support.
Urgent Visits by Paramedic
Our dedicated PCN Paramedic is available to respond to urgent care needs, helping to avoid unnecessary hospital admissions.
Multidisciplinary Team (MDT) Approach
We bring together GPs, nurses, pharmacists, social prescribers, and other specialists to plan and deliver joined-up care.
Care Coordinators
Our Care Coordinators act as a vital link between the care home, the clinical team, and families. They help organise appointments, follow up on care plans, and ensure continuity of care.
