Children under 16 need to be registered by their parent or guardian. You can do this by completing the registration from and childs immunisation history.

You will need to provide a form of ID to prove that you’re their parent or guardian.

Once completed you can hand in to the surgery or you can email to tmedicalcentre@nhs.net
